SCHOOL HEALTH
REQUIREMENTS
1. What does the law require?
All students (public & private schools) must meet certain health requirements,
including immunization, before school entrance.
2. Are there exemptions to the law?
Exemptions may granted for medical or religious reasons, provided the proper forms are
completed and returned to the school. Contact your school for information.
3. What are the three health requirements?
A physical exam is required once within one year prior to school entrance. A
tuberculosis (TB) clearance must be completed within one year before enrolling for
children 12 months of age and older. The TB test or chest x-ray must be performed in the
United States. And the following immunizations are required: diphtheria, tetanus
pertussis, polio, MMR (measles, mumps, & rubella), and Hib (haemophilus b for children
under 5).
4. What do I need to give to the school?
Your school will need a completed Hawaii School Health Record Form to prove that a
physical exam & TB test or chest x-ray were completed within a year before school
entrance, & that all immunization requirements have been met...and if your child has
not completed all the required vaccinations, a Signed Statement From Your Doctor to prove
that your child has begun the vaccination series and is in a waiting period for the next
dose in the series. All new students must also show proof that they tested negative in
either the TB test or chest x-ray before they can be admitted to school.
5. Where do I get the Hawaii School Health Record Form?
You can get a copy of the form from most doctors or any school.
6. What if my child is transferring from another state or
territory of the U.S.?
You will need to show proof that the health requirements have been met. Schools will
accept out-of-state records showing that the physical examination, tuberculin test, and
immunizations have been completed.
7. Where can I take my child for tuberculin skin testing and
immunizations?
In addition to seeing your private physician, services are available from public health
nursing clinics and non- profit clinics. Phone ASK-2000 for the location nearest you.
